- If your insurance has a co-pay amount per office visit, you are responsible for the “specialist copay” at the time of your visit.
- Your procedure is subject to your outpatient/ambulatory surgery center deductible and co-insurance amount at the time of your procedure.
- If you are having a procedure and we anticipate that you will incur out-of-pocket costs, you will receive an estimate via MyChart or mail approximately one week before your appointment.
- We will submit primary and secondary insurance claims on your behalf for each service you receive.
- You are responsible for payment of services, regardless of their reimbursement amount.
- To properly process your claims, the required information must be available and verified before your appointment. This includes details of your plan and any referrals that may be required. If this information is obtained after your visit, you are responsible for the balance.
- We accept payment by cash, check, money order, Visa, MasterCard, and Discover
